Ability
Assist provides a Care Coordination service designed to assist individuals to
manage the care component of their Care Plan to gaining access to attendant
care, medical, social, educational, and other services from different organizations
and providers.
WHO IS CARE COORDINATION FOR?
Care Coordination is for any individual
who has a Care Plan and who requires assistance to implement that plan and to
coordinate it on a day to day basis. An individual may have a Case Manager and
a Care Coordinator or just have a Care Coordinator once the Case Management
service has finished.

to support the individual to access services and supports
THE DIFFERENCE
BETWEEN CASE MANAGEMENT AND CARE COORDINATION
Care Coordination is where the care components of the Care Plan are coordinated by a person other than the Case Manager.
The Care Coordinator is not responsible for developing the Care Plan.
The Case Manager / Facilitator develops
the Care Plan and organises for implementation to commence.
