Ability Assist provides a Case Management service to assist individuals who have complex needs. Case Management will assist the individual to gain the outcomes and supports they need and to make the best use of resources available.  Case Management provides a framework to coordinate and provide health and social services for the individuals who require this service.

 

WHAT IS CASE MANAGEMENT?

 

“Case Management is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s health needs through communication and available resources to promote quality cost-effective outcomes.”  

WHO IS CASE MANAGEMENT FOR?

Any person who has a disability and has complex needs that are not readily addressed  by the individual themselves or their carer.
