Ability Assist provides a Case Management service to assist individuals
who have complex needs. Case Management will assist the individual to gain the
outcomes and supports they need and to make the best use of resources
available.
Case Management provides a
framework to coordinate and provide health and social services for the individuals
who require this service.

Any
person who has a disability and has complex needs that are not
readily addressed by the individual themselves or their carer.
