Ability Assist staff provides assistance to people with a disability, their families and / or their carers, allowing them to live as independently as possible within their community.  The support provided is individualised to meet the needs of the person receiving the service.

 

WHAT IS INDIVIDUALISED SUPPORT?

 

Individualised supports are especially designed for the person receiving the service to meet their support requirements and to assist them to achieve their life goals. Supports are flexible and tailored to individual needs and have a focus on community participation.  Support is provided by Ability Assist staff who are matched to the service user and who follow a Support Plan which has been developed by Ability Assist in conjunction with the service user and carer / family.

WHO IS INDIVIDUALISED SUPPORT FOR?

 

Any person who has support needs and who requires support for activities of daily living and/ or for accessing the community or doing the activities that they want to do.

An Assessment to determine day to day support requirement

Development of a Individual Support Plan

Support Staff to carry out the Support

Monitoring of implementation of the Support Plan

Feedback to the Carer/ Case Manager to let them know the progress
